Tapestry 360 Health is a mission-driven, patient-centered Federally Qualified Health Center (FQHC) delivering comprehensive primary care, behavioral health, and enabling services to a diverse and underserved population. As a Health Resources and Services Administration-designated health center operating under Section 330 of the Public Health Service Act, Tapestry serves as a critical safety-net provider, ensuring access to high-quality, equitable care regardless of a patient’s ability to pay.

With a workforce of approximately 250 employees and an annual operating budget of $44 million, the organization is advancing a deliberate strategy focused on financial sustainability, operational rigor, and long-term community impact. Key priorities include strengthening core infrastructure, expanding value-based care arrangements to align reimbursement with outcomes, and optimizing participation in the 340B Drug Pricing Program to reinvest resources into patient care. Through this integrated approach, Tapestry 360 Health is positioned to sustain and scale mission-critical services for the communities it serves well into the future.

Summary: The Community Health Coordinator promotes full access to Tapestry 360 Health services for specified populations on non-English-speaking patients and their families. They assist in language interpretation, care coordination, and bridging gaps in culture, communication and understanding between patients and healthcare providers. They will also support outreach and partnership activities with specified populations. This is a temporary, grant-funded position.

Essential Duties and Responsibilities:
  • Provides language interpretation to ensure efficient and accurate communication, ensuring patients understand care plans, medications, and follow-up.
  • Assists with medical referrals and follows through on completion. Coordinates patient care with various pharmacies, hospitals, and other providers.
  • Instructs patients on preparing for required procedures and tests. Accompanies patients to appointments. When necessary, advocates on behalf of high social- or medical-need patients.
  • Accurately transmits messages between patients and care teams.
  • Recruits, organizes, and serves as cultural navigator during group visits and workshops.
  • Completes assigned language-associated tasks efficiently and expeditiously.
  • Promotes patient independence.
  • Appropriately documents activities in patients' medical records.

Required Education and/or Experience:
  • At least one year of experience working in community programs or in a healthcare setting
  • English language proficiency


Preferred Education and/or Experience:
  • High School Diploma, GED or equivalent


Language Skills:
  • Fluency in Dari/Pashto or Rohingya/Burmese highly desired
  • Medical interpretation certification desired

Position Type/Expected Hours of Work: 20 hours per week


Travel: Must be able to access and use public transportation with patients, when necessary.

Benefits: This position is not eligible for company benefits.
Pay Transparency: The salary range for this position is $22.69 hourly. Actual compensation will be determined based on factors including, but not limited to, relevant experience, qualifications, internal equity, and organizational needs.
