Job description

TITLE OF IMMEDIATE SUPERVISOR: Director of Clinical Services

RISK OF EXPOSURE TO BLOODBORNE PATHOGENS - HIGH


DUTIES

To provide nursing care is in accordance with the pediatric client's plan of care.

RESPONSIBILITIES

1.    Conducts initial and ongoing comprehensive assessments of the client’s needs.

2.    Comprehensive health and psychosocial evaluation, monitoring of the client's condition, health promotion and prevention, coordination of services, teaching and training activities.

3.    Initiates the plan of care and revises it as necessary.

4.    Provides all services requiring specialized pediatric nursing skills.

5.    Educates the client and family regarding the disease process, self-care techniques and prevention procedures.

6.    Initiates and maintains appropriate preventive and rehabilitative nursing procedures.

7.    Prepares clinical and progress notes; evaluates the effectiveness and outcomes of care.

8.    Coordinates services with other departments and other appropriate care givers.

9.    Analyzes and provides chart assessments for case conferences.

10. Acts as a liaison with physicians, hospitals and specialty organizations and provides them with information and instruction.

11. Supervises LPNs and paraprofessionals providing services to the client.

12. Plans for the client's discharge from the program.

13. Participates in in-service programs.

JOB CONDITIONS

1.    Must be able to access clients’ homes which may not be routinely wheelchair accessible.

2.    Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.

3.    Physical activities will include, walking, sitting, stooping, and standing and minimal to maximum lifting of clients and the turning of clients.

4.    The ability to communicate both verbally and in writing is required as frequent communication by telephone and in writing is involved.

EQUIPMENT OPERATION

Thermometer, B/P cuff, glucometer, penlight, hand washing materials

COMPANY INFORMATION

Has access to all client medical records, personnel records and client financial accounts which may be discussed with the Director of Clinical Services

QUALIFICATIONS

1.    Must be a graduate from an accredited School of Nursing.

2.    A Bachelor’s degree is preferred, but not required.

3.    Must be licensed in the State of Maryland as a Registered Nurse.

4.    One year Pediatric Nursing experience is required, preferably in a Home health Agency; should also have expertise in any specialized nursing procedures and equipment used for infants, children and adolescents. 

5.    Must have current CPR certification.

6.    Must have a driver’s license and a reliable means of transportation to assure timely arrival at client’s homes.

7.    Must have a criminal background check.
