$133,600.00 - $193,750.00/Annual Actual pay is determined based on job-related factors such as relevant experience, education, credentials, skills, internal equity, and business needs.
Overview
The RMG Ambulatory Pharmacist provides patient-centered, safe, effective and comprehensive medication management support at the population health and individual patient level. This professional role serves as a pharmacy resource to the Riverside Medical Group (RMG) to provide practice-related interventions, education, and training. Leading process improvement projects and participation in the quality management program by gathering and analyzing data, implementing interventions, monitoring outcomes, facilitating pharmaceutical care integration, and ensuring regulatory compliance is expected. The RMG Ambulatory Pharmacist demonstrates integrity and competency and performs all functions of the Clinical Pharmacist.Performs all duties per state board regulations. Maintains professional decorum at all times.
What you will do
  • Assist with formulary/protocol development and management related to specialty area(s)
  • Participate in direct patient care clinics, providing actionable medication related changes/monitoring to providers. Provide pharmaceutical care appropriate to the patient age and medical condition. Recommend evidence-based care. Collect data regarding recommended interventions to assess overall impact.
  • Provide general and patient specific drug information to patients, physicians, nurses and, and other health care professionals.
  • Routinely interact with physicians, nurses, and other health care professionals to provide optimal pharmaceutical care to patients.
  • Support and participate in pharmaceutical related research in area of specialty
  • Participate in population health activities with a focus on medication related metrics

Qualifications
Education
  • Bachelors Degree, Pharmacy (Required)
  • Doctoral Degree, Pharmacy (Preferred)

Experience
  • 2 years Clinical Pharmacy experience (Required)
  • PGY-1 Residency (Preferred)
  • PGY-2 Residency, Ambulatory Care (Preferred)

Licenses and Certifications
  • Licensed Pharmacist - Virginia Department of Health Professions (VDHP) Upon Hire(Required) and
  • Board of Pharmacy Specialties (BPS) - Board of Pharmacy Specialties Ambulatory Care (or equivalent as approved by hiring manager) within 3 Years(Required)
