JOB TITLE: Home Health Aide [including STNA, CNA, HHA, PCA, DSP/DODD]
  • WHO SHOULD APPLY:

JOB LOCATION: West Lafayette, Ohio; Coshocton, Ohio; Coshocton County, Ohio and surrounding areas
CARE LOCATION & TRAVEL:
  • Typical Travel Requirements: MUST be willing to serve clients throughout Coshocton County, OH (~30-40 min radius).
  • 45-60 min radius is preferred but not required ("road warriors" receive priority)
    • May also be required for anyone seeking Full-Time schedules, plus adjacent counties (not required)
  • Reliable transportation is required.

EMPLOYMENT TYPE: Part-time + Long-term employment | Potential for Full-time is optional
  • HCN's priority is to create long-term employment opportunities for home care & home health professionals seeking Part-time or Full-Time positions, which ensures that our clients receive consistent, familiar, and exceptional care.
  • Part-Time: ~10-20 hrs/week (up to 31/wk)

COMPENSATION:
  • Certified Caregivers with qualified experience start at $16 - $19 per hour
  • Non-Certified or First-Time Caregivers start at $15 - $17 per hour

Position Reports to: RN Case Manager / Program or Regional Administrator
POSITION SUMMARY:
We are seeking a highly-skilled, compassionate, and professional Home Health Aide (STNA or CNA highly desired | experienced HHA, CHHA, DSP, or PCA will also be considered) to join our home health agency and provide hands-on personal care and support to clients across Coshocton County, including West Lafayette, OH, Coshocton, OH, and surrounding areas. A successful Home Health Aide at HCN provides personal care services under the direction of the Registered Nurse or Therapist in their region. You will be assigned to specific clients by the Registered Nurse or other appropriate professionals and perform services for clients as necessary to maintain personal comfort and safety in their home.
As a Home Health Aide, you will:
  • Deliver direct patient care and support, following the established plan of care, to pediatric, adult, and geriatric patients.
  • Measure and record vital signs and other specified metrics as instructed.
  • Assist with personal care tasks and activities of daily living (e.g., bathing, oral hygiene, nail & skin care, shaving, dressing).
  • Perform household tasks essential to health care and living at home (e.g., meal preparation/feeding, laundry, light housekeeping).
  • Assists patients in taking medications under the direction and supervision of the Registered Nurse. This includes opening a container or pill caddy & handing it to the patient but does not include administration of the medications.
  • Identify and report any abnormal findings or changes in the patient's condition.
  • Help with therapy exercises under supervision of our therapists (e.g., range of motion exercises, ambulation assistance).

Responsibilities Include:
  • Personal Care & Activities of Daily Living (ADLs) including safe transfers plus mobility and positioning support
  • Support safe use of assistive devices (walkers, wheelchairs, lifts)
  • Nutrition & Meal Support
  • Homemaking & Environmental Support (IADLs) including light housekeeping, errands, and tasks required to maintain a clean, safe, and organized home environment
  • Community Engagement & Appointment Support, Coordination, and (in approved situations) Transportation
  • Basic Health and Safety Monitoring
  • Care Plan Implementation
  • Dementia & Behavioral Support (if applicable)
  • Pro-active observation and excellent electronic documentation
  • Client Rights, Dignity & Confidentiality
  • Total Care Team Collaboration
  • Ongoing Training & Certification Requirements

Qualifications & Requirements
Job-Related (non-negotiable):
  • Minimum of 1 year paid in-home care or caregiving experience (STNA/CNA certification in lieu of experience will be considered)
  • Successful completion of all agency-provided HHA/PCA orientation, skills tests and competency evaluations.
    • Willingness and timely compliance to maintain all ongoing Training & Certification Requirements
  • Active CPR + FA certification and documentation of both (+ proof/documentation) - or willingness to complete through HCN within 2 weeks / prior to hire. Medication Administration Certification is highly preferred.
  • Willingness and aptitude to perform light housekeeping in addition to patient care services as needed.
  • Demonstrated ability to read, write, and follow a written Plan of Care, incorporating attention to detail in following instructions, documenting notes, and communicating with our agency.
  • Ability to perform all physical demands of home care on a regular basis, including:
    • Frequent: standing, walking, stopping, ending
    • Infrequent: squatting/kneeling, balancing
    • Pushing and handling weight/force: 1-20 lbs. frequently, 20-30 lbs. occasionally, and 30-50 lbs. infrequently
    • Lifting and carrying: 1-20 lbs. frequently, 20-30 lbs. occasionally, 30-50 lbs. infrequently

  • Must be at lease eighteen (18) years of age, with an active Driver's License / State ID + US Work Authorization.
  • Must have reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients

Preferred Skills & Qualifications (not required):
  • 3-5 years of paid experience in a home health or caregiving setting.
  • Paid experience with a Home Health Agency.
  • Experience with:
    • G-tubes / feeding support
    • Hoyer Lifts
    • Seizure protocols
    • Behavioral health or crisis response
  • High School diploma or GED
  • Formal certification or training program: STNA (or 2026 graduation date) highly preferred; CNA or CHHA still encouraged to apply.
  • Specialized certification or training program, such as DODD-DSP, Med Certification, EMR/EHR, etc.
  • EMR/EHR experience and proficiency (ContinuLink highly desirable).
  • Familiarity with Medicaid waiver services
