Job description

JOB TITLE: Pediatric HHA/CNA/STNA (Home Health Aide)
  • Minimum of 1-year paid experience with pediatric patient is required, regardless of certifications.
  • STNA or CNA is highly desirable.
  • HHA, PCA, DSP/DODD with 2+ years pediatric caregiving experience will also be considered.

LOCATION + TRAVEL: Holland, OH - Lucas County
  • Willingness to travel to/from client in Holland, OH (Lucas County) is required.
  • Willingness to travel to clients up to at least a 30-minute radius (from your home) is required if additional hours are needed, primarily consisting of surrounding areas in Lucas and/or Wood County (not required).

SCHEDULE:
  • Mondays: 8:30 AM - 4:30 PM (8hr shift x 1 day) | Tuesdays - Fridays: 4:00 PM - 8:00 PM (4hr shift x 4 days)
  • Requires blend of days + evenings (exact start + end times are slightly flexible)

EMPLOYMENT TYPE: Part-Time [24hrs/wk] with desire for Long-term commitment (2+ years at HCN)
  • Must be willing and able to start immediately with this PT case (24hrs/wk)
  • 30-day build-up to additional Part-Time or Full-Time (32-40 hrs/wk) hours is available if desired (not required)

Home Care Network, Inc. is URGENTLY SEEKING an experienced, compassionate, and professional Pediatric Home Health Aide (STNA or CNA highly desired | experienced HHA, CHHA, DSP, or PCA will also be considered) to join our home health agency and provide hands-on personal care and support to a pediatric client in Holland, OH.
Responsibilities
  • Personal care, hygiene and Activities of Daily Living (ADLs) including safe transfers, and mobility/positioning support
  • Support safe use of assistive devices (walkers, wheelchairs, lifts) - as needed
  • Nutrition & Meal / Meal Prep Support
  • Homemaking & Environmental Support (IADLs) including light housekeeping, errands, and tasks required to maintain a clean, safe, and organized home environment
  • Community Engagement & Appointment Support, Coordination, and (in approved situations) Transportation
  • Basic Health and Safety Monitoring
  • Care Plan Implementation
  • Dementia & Behavioral Support (if applicable)
  • Demonstrated ability to follow a written Plan of Care

  • To Start: this is available as a part-time, weekday position requiring consistent availability of 20-30 hrs/wk in day + evening hours.

  • Local travel within Lucas County | Expanded cases/hours may include adjacent Counties

Requirements / Non-Negotiables
  • Minimum of 1 year paid in-home care or caregiving experience of pediatric patient(s)
  • STNAs and CNAs highly desired (or HHA/PCA/DSP with 2+ years consistent experience)
  • Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid).
  • Active CPR + FA certification and documentation of both
  • Character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful
  • Willingness and ability to perform required physical demands (see below)
  • Consent + Ability to pass a State & Federal background check (including fingerprints)
  • 18yo + active Driver's License / State ID
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius

A successful Home Healthcare Aide provides personal care services under the direction of the Registered Nurse or Therapist in their region. You will be assigned to specific clients by the Registered Nurse or other appropriate professionals and perform services for clients as necessary to maintain personal comfort and safety in their home.
Report to: RN Case Manager / Administrator / Director of Nursing
FULL REQUIREMENTS + QUALIFICATIONS:
  • Successful completion of a formal certification training program and/or a written skills test and competency evaluation. CHHA, CNA, and STNA accreditation desirable but not required.
  • Meet the training and/or certification requirements of the state in which the home health aide services are being provided. This includes specifications of contracted agencies for which services are being provided.
  • Minimum of 1 year paid in-home care or caregiving experience.
  • Active CPR + FA certification and documentation of both.
  • Be at least eighteen (18) years of age.
  • Current driver's license, good driving record, and/or reliable transportation is required.
  • Exceptional on-time and attendance record.
  • Consent + Ability to pass a State & Federal background check (including fingerprints).
  • Demonstrated ability to read, write, and follow a written Plan of Care.
  • Good interpersonal skills and a passion for caring for those in need.
  • Willing and able to regularly perform all physical demands of home care (including lifting/carrying up to 30-50 lbs. occasionally).
  • Willingness and aptitude to perform light housekeeping in addition to patient care services as needed.

ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY:
Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist.

  • Assists in the patient in taking medications under the direction and supervision of the Registered Nurse. This includes opening a container or pill caddy & handing it to the patient, but does not include administration of the medications.
  • Knows and follows infection control policies, including proper hand washing techniques.
  • Reports any observed or reported changes in the client's condition and/or needs to the Registered Nurse.
  • Documents care provided and completes the forms required for the client's records. Completes the appropriate records to document cares given and pertinent observations.
  • Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager.
  • Demonstrates safe practice in the use of equipment. Does not use equipment until orientation has been provided. Notifies supervisor of educational needs.
  • Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.
  • Treats patients and families with respect. Maintains confidentiality in all aspects of the job.
  • Accepts and fulfills assignments with the agency. Exercise good judgment in accepting assignments.
  • Participates in required in-service programs to meet compliance requirements. Identifies learning needs to agency supervisor.
  • Performs other related duties and responsibilities as deemed necessary
