A brief overview of Responsibilities:
  • Provides skilled nursing care to homebound patients in accordance with the established Plan of Care, developing and implementing individualized treatment plans while monitoring and reporting changes in condition. Coordinates and manages multidisciplinary services to ensure continuity of care, including admissions, transfers, and discharges, and collaborates closely with the interdisciplinary team. Educates patients and families on health maintenance and disease prevention, completes timely electronic documentation in Homecare Homebase, and offers guidance and emotional support throughout the care journey.
Qualifications:
  • Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
  • At least one year of home health experience preferred
  • Graduate of an accredited nursing school.
  • Must have a valid driver's license and a reliable car available during work hours.
  • OASIS and HCHB experience PLUS!
