Job description

Position Summary

The Personal Care Provider (PCP) / Certified Nursing Assistant (CNA) delivers non-medical home care services to clients in their residences. This role focuses on assisting clients with activities of daily living, ensuring a safe home environment, and providing compassionate companionship in accordance with each client’s individualized care plan and agency policies.


Minimum Qualifications

  • Must be at least 18 years of age

  • Able to lift a minimum of 25 pounds

  • Free from communicable diseases, open skin lesions, or other health conditions that may interfere with job performance or client safety

  • Completion of required home care training and successful competency evaluation prior to providing direct client care

  • Ability to meet physical and mental demands of the position


Essential Job Duties

Home Safety & Environment

  • Observe and help maintain a safe and secure home environment

  • Report mechanical, structural, or safety concerns to management

Household & Errand Support

  • Assist with meal preparation, light housekeeping, laundry, and dishwashing

  • Perform shopping and errands as outlined in the care plan

  • Assist clients with appointments and community activities

Companionship & Emotional Support

  • Provide companionship, conversation, and social interaction

  • Offer emotional reassurance and encouragement

  • Promote mental stimulation through reading, writing, and engaging activities

Personal Care & Activities of Daily Living (ADLs)

  • Assist with bathing, dressing, grooming, oral care, hair care, shaving (electric or safety razor only), and general nail care

  • Assist with toileting, incontinence care, pericare, bedpans, urinals, and commodes

  • Empty urinary collection devices and ostomy bags as outlined in the care plan

  • Assist with feeding and hydration

  • Provide assistance with exercise and mobility as specified in the service plan

  • Assist with ambulation for weight-bearing clients using assistive devices

  • Perform transfers as outlined in the care plan

  • Provide general skin care to unbroken, healthy skin

Medication & Health Support

  • Provide medication reminders and prompting only

  • Assist with limited respiratory support, including temporary removal and replacement of a cannula for grooming or hygiene

  • Provide protective oversight and standby assistance as required

Documentation

  • Complete visit documentation for each service provided

  • Documentation must include date, time in/out, services performed, and client signature
