What You'll Do
  • Conduct patient evaluations and develop individualized treatment plans
  • Provide therapeutic exercises, functional training, and pain management interventions
  • Educate residents and families on safe use of equipment and exercise techniques
  • Collaborate with interdisciplinary care teams to promote resident progress and independence
  • Monitor patient outcomes and adjust treatment plans as needed
  • Complete accurate and timely documentation and billing
  • Maintain compliance with all state and federal regulations

Qualifications
  • Licensed Physical Therapist in Oregon or eligible for Oregon licensure
  • Graduate of a CAPTE-accredited Physical Therapy program
  • Strong communication, clinical reasoning, and problem-solving skills
  • Ability to work collaboratively in a team-based environment
  • Skilled nursing or post-acute experience preferred, but not required
