Practice Setting: Free-Standing Cancer Center 
Reason for Coverage: Loss of provider
Call Schedule: No call during locum period; call expected if placed perm; call schedule is 1 week out of every 6-8 weeks
Call Ratio: No call during locum period; call expected if placed perm; call schedule is

Patient Population for Required Coverage: Adult, Geriatric
What Percentage of the Practice is Hematology %, Oncology %, Internal Medicine %: 65% benign hematology, 35% malignant oncology, 0% internal medicine
Number of Physicians in Practice: 7
