Job Title: Home Health Registered Nurse – RN

Location: Danville, VA & Surrounding Areas (In-Home Patient Care)

Schedule: Flexible Scheduling Available (Full-Time, Part-Time & PRN Opportunities)

Pay: Competitive per visit rates + mileage reimbursement


Job Overview:

We are currently seeking a compassionate and skilled Home Health Registered Nurse – RN to provide personalized, one-on-one patient care throughout Danville, VA and the surrounding areas. This position is ideal for an RN seeking flexibility, autonomy, and the opportunity to make a meaningful impact while caring for patients in the comfort of their homes.

As a Home Health Registered Nurse – RN, you will provide skilled nursing care to patients recovering from illness, injury, or chronic conditions while collaborating closely with physicians, families, and interdisciplinary care teams. This role requires travel to patient homes throughout the service area.


Responsibilities of Home Health Registered Nurse – RN:
  • Provide skilled nursing care to patients in a home setting
  • Complete patient assessments and monitor patient progress
  • Develop, implement, and update individualized care plans
  • Educate patients and families on medications, treatment plans, diet, exercise, and home safety
  • Coordinate care with physicians, LPNs, CNAs, and other healthcare team members
  • Maintain accurate and timely documentation
  • Identify changes in patient condition and communicate updates to physicians and care team
  • Coordinate patient discharge planning and ensure care goals are met

Qualifications of Home Health Registered Nurse – RN:
  • Graduate of an accredited nursing program
  • Active RN license in the state of Virginia
  • Minimum of 1 year of nursing experience (home health preferred)
  • Current CPR certification and TB screening
  • Valid driver’s license, reliable transportation, and auto insurance
  • Willingness to travel throughout the service area
  • Strong clinical judgment, communication, and organizational skills
  • Compassionate and patient-focused approach to care
