What You'll Do

As the Planetree Program Manager, you'll serve as the organizational champion for Logan Health's person-centered care philosophy by:

  • Leading and advancing Planetree and person-centered care initiatives across Logan Health.

  • Partnering with leaders to align Planetree goals with organizational strategic priorities.

  • Developing educational programs, resources, and training that foster a culture of compassion, empathy, and excellence.

  • Measuring program effectiveness, analyzing outcomes, and identifying opportunities for continuous improvement.

  • Supporting departments as they implement person-centered practices and helping remove barriers to success.

  • Gathering and utilizing patient and employee feedback to enhance experiences and improve care delivery.

  • Collaborating with internal teams-including Marketing and the Foundation-to promote and celebrate person-centered care initiatives.

  • Sharing stories, successes, and outcomes that highlight the impact of Planetree throughout the organization.

  • Building relationships with Planetree affiliates and staying current on national best practices.

  • Representing Logan Health through presentations, community engagement, and organizational events.

Required Qualifications

  • Bachelor's degree in a related field or an equivalent combination of education and experience.

  • Minimum of five (5) years of healthcare experience.

  • Strong organizational, project management, and critical thinking skills.

  • Excellent written, verbal, and interpersonal communication abilities.

  • Ability to collaborate across all levels of the organization while maintaining professionalism and confidentiality.

  • Proficiency with Microsoft Office and the ability to quickly learn new technology.


Preferred Qualifications

  • Previous experience working with Planetree programs or other person-centered care initiatives.
