This position is entirely remote, allowing you the flexibility to work from any location while serving our diverse patient base. You will engage with colleagues and external healthcare providers to create and follow integrated care plans for our patients.

• Conduct thorough assessments of patients’ mental health statuses, evaluating emotional, behavioral, and social functioning.
• Provide individualized psychotherapy services for patients with various mental health conditions, including anxiety, depression, and PTSD.
• Develop and implement evidence-based, culturally competent treatment plans that are customized to address each patient's unique needs and circumstances.
• Monitor patient progress through scheduled follow-ups, adjusting treatment plans as needed based on ongoing assessments and feedback.
• Collaborate closely with multidisciplinary teams, including psychiatrists and primary care physicians, to ensure holistic care for patients.
• Educate patients and their families about mental health conditions, treatment modalities, and community resources to enhance understanding and self-management.
• Maintain ethical standards and confidentiality in all aspects of practice, as per state and national regulations.

The patients you will serve come from various backgrounds, reflecting the rich diversity of the Santa Ana community. • A Master's degree in Social Work from an accredited institution is required.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
