This is a full time, direct care position. The primary responsibilities of the Extender are to:
  • Perform general outreach, engagement, and follow-up with members
  • Spanish language proficiency preferred
  • Coordinate services/appointments
  • Engage clients in health promotion activities
  • Share information with Care Manager on the member's circumstances
  • Make and track referrals
  • Participate in case conferences
  • Support the care manager in assessing and addressing unmet health-related resource needs.

Requirements
Must be a be at least 18 years of age, have a high school diploma (or equivalent)
1-2 years paid experience preferred within behavioral/mental health field.
Spanish speaking/bilingual preferred.
