PRIDE Health is seeking a travel nurse RN Home Health for a travel nursing job in Orange Park, Florida.

Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.

Compensation:

  • Travel Pay: $2,226.00 gross per week

Location: Orange Park, FL, 32073

  • Shift: 8 Hours/Day, Monday to Friday, 09:00 AM to 05:00 PM
  • Guaranteed Hours: 40 Hours/Week
  • Contract Duration: 14 Weeks

Key Responsibilities:

  • Conduct comprehensive home health assessments, complete OASIS documentation, develop and implement individualized Plans of Care (POC), and provide skilled nursing interventions while ensuring accurate clinical documentation and regulatory compliance using Homecare Homebase (HCHB).
  • Coordinate patient care with physicians, caregivers, and the interdisciplinary team; educate patients and families on disease management, medications, and home safety; supervise Home Health Aides (HHA/LVN/STNA); and deliver high-quality, patient-centered care in accordance with agency and payer guidelines.

Required Qualifications

  • Active Registered Nurse (RN) license in the state of practice.
  • Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma program.
  • Minimum 2+ years of Home Health experience.
  • Minimum 1–2 years of RN clinical experience.
  • Homecare Homebase (HCHB) experience – Required.
  • OASIS documentation experience – Required.
  • Current CPR/BLS Certification.
  • Valid driver's license.
  • Reliable transportation with current auto insurance.
  • Ability to travel locally for patient visits.
