Job description

Are you passionate about making a tangible difference in the lives of individuals facing mental health challenges? Northwestern Counseling & Support Services, Inc. is seeking a Stabilization Support Specialist to be part of our dynamic Stabilization Support Team. Our team provides intensive, individualized services ranging from case management to 1:1 skill coaching and family/parenting support to clients experiencing clinically acute needs. Our mission is to help clients stabilize so they can safely remain in their homes and communities, or transition successfully after a higher level of care.
As a Stabilization Support Specialist, you'll play a vital role in supporting clients and families through challenging times. You'll provide flexible, strengths-based, and client-centered services that empower individuals to build resilience, develop coping skills, and thrive within their communities.
Key Responsibilities:
  • Deliver intensive, individualized support to clients experiencing mental health challenges.
  • Provide family and parenting support to strengthen home resilience.
  • Facilitate 1:1 skills coaching to promote emotional regulation, daily living success, and community integration.
  • Coordinate care and provide case management with internal teams and community partners.
  • Support clients in stabilizing within their community or transitioning successfully from higher levels of care.
  • Maintain accurate, timely, and thorough documentation.
  • Actively participate in team meetings, collaborative problem-solving, and ongoing professional development.
