Care Partner, Aged Care. Permanent Full Time - Sunshine Coast

About the Role

As a Care Partner, you will lead the delivery and continuous improvement of our Aged Care services across the Sunshine Coast. In this role, you will manage client referrals, support individuals as they commence services, and regularly review and update care plans to meet clients changing needs. You will coordinate home visits to ensure clients receive high-quality, person-centred care, while fostering strong relationships with clients, families, and key stakeholders.

You will also contribute to the effective coordination of the program by reviewing, enhancing, and maintaining operational and administrative processes. With a strong focus on client experience, you will help ensure clients and their families feel supported and confident in the services they receive, while supporting the continued growth and success of our Aged Care programs.

We currently have a permanent full-time position within the Sunshine Coast, with a remote working model, travelling across the region to visit clients in their homes as required. An opportunity for a motivated professional to take ownership of the role and further develop their career in the aged care sector.

Key Responsibilities

  • Undertake a comprehensive client assessment, risk assessment, client profile and care plan aligned to the relevant program
  • Conduct home visits and ensure care plans are monitored and reviewed on a regular and ongoing basis, and in response to the changing needs of clients.
  • Ensure care plans are developed in partnership with the client/representative and other relevant stakeholders.
  • Work collaboratively with internal teams, including Rostering and Practice Leads, to meet client needs
  • Develop strategies and/or broker services to address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities
  • Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community

Skills & Experience

  • Demonstrated experience in all aspects of case management including comprehensive assessment, planning, provision and evaluation of services for individuals and their carers, including people with culturally and linguistically diverse backgrounds.
  • Demonstrated experience in the creation of care plans, budgets, reports, correspondence and general documentation relevant to programs.
  • Confidence in utilising a variety of technology including mobile devices and computers
  • Proficiency with Microsoft Office, Outlook and database systems
  • Ability to effectively build relationships with diverse stakeholders
  • Strong communication skills
  • Ability to multi-task and work autonomously in a fast-paced environment
  • A current unrestricted Australian Drivers Licence, reliable vehicle and a willingness to travel
