Therapeutic Case Manager, Newark, Miller's Run, Sutton, and Burke Schools
As the NKHS Therapeutic Case Manager you will be based out of the Cornerstone School day treatment program integrating the support services for students. This is an exciting opportunity to collaborate, backed by a strong clinical team providing individual and family supportive interventions. Collaborate with school personnel to develop treatment and transitional plans for students in Caledonia County for Cornerstone school and the Kingdom East Collaborative. Opportunity to be deeply connected in a small school setting while gaining valuable professional experience.
Schedule follows typical school calendar and includes summer program. 40 hours/week, 7:30 am - 3:30 pm. Follows school calendar plus summer programming for approximately 5 weeks. This role will travel between Miller's Run, Newark, Sutton, and Burke Schools.

Responsibilities: Therapeutic Case Manager
  • Have or develop knowledge of local, state and regional community resources.
  • Meet with or have telephone contact with selected children and parents for evaluation, supportive interventions, crisis management and communication of significant information.
  • Responsible for the coordination of clinical treatment team meetings..
  • Responsible for the ongoing development and implementation of transition plans with referring agencies and/or school systems, and potential post program placements..

QUALIFICATIONS
  • Master's degree in human service field preferred; will consider Bachelor's with relevant experience.
  • Experience with emotionally/behaviorally challenged children and youth highly desirable.

SALARY & BENEFITS
  • $23.75 - $29.03 per hour, depending upon education and experience
