Job Summary and Qualifications
  • ​​Assumes responsibility for the clinical coordination and consultation regarding care across the continuum for all stroke and sepsis patients.
  • Consults and partners with nursing leadership as needed to achieve desired patient outcomes
  • Assesses educational needs of staff and assists in the development of educational programs, staff education materials, and community education as needed
  • Maintains and ensures compliance of continuing stroke education for providers and nursing staff
  • Ensures evidence-based screening and treatment protocols are the standard of care within the facility
  • Provides appropriate feedback and coaching to staff based on data analysis and observation
  • Ensures accurate and timely collection of clinical data, verifying and updating data as needed
  • Shares prepared data with key stakeholders in an understandable and meaningful context to motivate adherence to protocols and continuous improvement
  • Co-chairs the Stroke Multidisciplinary Committee and chairs the Sepsis Committee Meetings
  • Serves as a resource to staff, physicians, patients, and families
  • Completes stroke and sepsis core measure abstraction, ensuring core measure requirements are met and updated for Joint Commission requirements
  • Performs a variety of technical and specialized tasks involved with the concurrent and retrospective evaluation of patient care
  • Utilizes quality improvement tools, including root cause analysis, to investigate fall outs and generate action plans
  • Provides assistance to all hospital departments in implementing and sustaining their quality improvement processes related to stroke and sepsis.
  • Assumes responsibility for meeting all regulatory requirements, including Joint Commission Primary Stroke Certification as well as maintaining a state of survey readiness for this program.
  • Participates in division-wide collaboration to improve patient outcomes
  • Demonstrates working knowledge of AIDET, utilizing with each patient interaction
  • Regular, reliable attendance is required to ensure the highest level of patient care
  • Follows hospital safety and quality standards and expectations, as it pertains to job performance and patient care

     What qualifications you will need:

    • Advanced Cardiac Life Spt must be obtained within 6 months of employment start date
    • Basic Cardiac Life Support must be obtained within 30 days of employment start date
