Job Summary and Responsibilities

As a Clinical Coordinator RN, you will be a central figure in patient care, seamlessly navigating the healthcare journey to achieve optimal outcomes and an exceptional patient experience.
Every day, you will strategically assess, plan, and facilitate comprehensive care across the continuum, expertly advocating for patients while collaborating with physicians, nursing, departments, insurers, and post-acute providers to ensure timely, high-quality transitions.
To be successful in this role, you will possess strong clinical acumen, exceptional communication and advocacy skills, and a strategic mindset, all driven by a passion for optimizing patient care across every touchpoint.

  • In conjunction with the Director, the Clinical Coordinator is responsible for the day to day operation of the agency by providing supervision and direction to clinical and non-clinical staff, through MDE ordering, MD signatures, consultations, phone calls, continued authorization for ongoing visits, facilitates resolution of client clinical issues, staff questions, assist utilization with billing questions/resolutions and hospice responsible meds, DME and labs
  • Responsible for assignment and coordination of services for all admits, through face to face, DME, medication access, medical records and family consults
  • Monitors staff productivity within HHRG (fiscal) while knowing when to increase visits to achieve greater outcomes (quality), with oversight of therapy and nursing visit numbers and uUtilizing Clinical judgment to adjust HHRG calculation
  • Responsible for assuring that clinical documentation meets accepted standards of care, regulatory, operational and financial requirements, with quarterly and prn Clinical record review
  • Facilitates coordination of care through case conferences and other activities to ensure interdisciplinary care is occurring, by leading/facilitates meetings, monitors individualized care plans and assures conversations regarding, visit numbers, therapy needs, discharge plans and quality are being voiced.
  • Performs OASIS and HIS submissions as regulated.
