Hospice Transition Nurse
Position Overview
The Hospice Transition Nurse is a resource Registered Nurse to patients, family members and staff of hospitals, physicians, and nursing homes. They are responsible for sustaining and increasing referral sources from the hospitals and nursing homes and other community sources. The Hospice Transition Nurse uses their knowledge of St. Croix Hospice programs and services to provide education to these referral sources. You will have responsibility for about 5-6 hospitals, handling admissions, GIP patients, and ensuring a smooth transition for patients and families.
Essential Functions and Skills
  • Performs as a case manager for patients in the hospital including managing GIP patients when appropriate. Acts as a resource to the hospital and able to navigate hospital EMR systems to locate appropriate documentation needed to support hospice admission.
  • Conducts initial goals of care meetings with patients and families to inform them about St. Croix Hospice programs and services.
  • Collaborates with business development in the community by providing education to facilities and other referral sources regarding Hospice programs and services.
  • Performs follow-up meetings and phone calls from information visits.
  • Sustains and increases referral sources with hospitals, nursing homes and other community sources.
  • Documents visits and phone calls with patients, families, and referral sources.
  • Assists with transition of care for patients out of the hospital that are being transferred home or admitted onto care with St. Croix Hospice including coordination with admission nurse and Hospice Medical Director, coordination of comfort medications and DME needs, and completion of hospice admission consents.
  • Mentors and trains new Registered Nurses as appropriate for high acuity patient needs.
  • Attends Interdisciplinary Care Meetings regarding patients and families in the hospital.
  • Establishes and maintains relationships with hospital Case Mangers, Social Workers, and Physicians including attending any necessary daily rounds.
  • Other duties may be assigned or required for the performance of this position up to and including completing routine visits, admissions and other clinical tasks.

Requirements/Qualifications
  • Must have current Registered Nurse (RN) license in state(s) of practice.
  • Ability to pass DHS background study.
  • Valid State Driver's License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.

Preferred
  • Graduate of an accredited school of nursing required. Associate degree required. Bachelor's degree preferred.
  • 1+ years of recent experience in hospice required.
  • 2+ years' experience in Nursing, preferred

Physical Requirements
The physical requirements described are representative of those that must be met to successfully perform the essential responsibilities of this position. Reasonable accommodation may be made.
  • Pushing/Pulling and Lifting/carrying up to 30 pounds. Occasionally up to 50 pounds.
  • Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting.

The starting salary range for this role is $95,000-$100,000 annual base salary. An employee's actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
