Job description

Job Summary

The Case Manager participates in outreach activities to identify and enroll SSVF-eligible veterans in case management services, ensuring timely access to safe, permanent housing and essential supportive services. The position utilizes a Housing First Model to develop and implement Housing Stability Plans that promote household stability and self-sufficiency.

Key Responsibiliities

  • Conduct full assessments with applicants to enroll SSVF-eligible veterans in case management for a minimum of 90 days, documenting all activities and events
  • Ensure rapid entrance into permanent safe housing or maintain stable housing to prevent homelessness
  • Mediate, advocate, and leverage support for eligible veterans, referring them to healthcare navigation, legal, employment, financial, and other services as needed
  • Identify and connect veterans to community resources for overall well-being, including emergency housing needs
  • Maintain accurate records of all resources provided, including temporary financial assistance, and manage client files safely while preserving confidentiality
  • Represent Baker Ripley and the Veteran Services team professionally in internal and external meetings and events
  • Collaborate with other agencies to provide a comprehensive case management process
  • Perform other duties as assigned

Education

Bachelor’s degree preferred; high school diploma or GED required

Experience

2+ years of case management experience; 1–3 years minimum; 2+ years social services experience preferred

Skills

Proficiency in Excel, clear and accurate correspondence/reporting, ability to work in a professional, client-focused environment

Licenses/Certifications

Valid driver’s license; experience working with veterans or in similar outreach roles is a plus

Knowledge, Skills, and Abilities

  • Ability to develop Excel spreadsheets using formulas
  • Ability to write clear and accurate correspondence and reports
  • Ability to effectively present information and respond to questions both orally and in writing in a manner that is understandable to others
  • Ability to calculate figures and amounts
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form
  • Excellent interpersonal skills and successful experience working in teams
