Openwork is hiring an experienced Case Manager / Housing Facilitator to support young adults (ages 1824) as they work toward stable housing and independent living. This is a rewarding opportunity to make a meaningful impact by helping individuals access housing, community resources, and ongoing support.

What You'll Do
  • Assist clients with housing searches, lease applications, and move-ins
  • Help participants with housing vouchers and required documentation
  • Conduct assessments and develop individualized service plans
  • Provide ongoing case management and support
  • Maintain accurate case notes and client documentation
  • Connect clients with community resources and partner agencies

What We're Looking For
