The Senior Case Manager ensures continuity and quality of services by managing temporary and complex cases, particularly during staffing transitions. This role provides person-centered case management services to individuals with disabilities, mental health conditions, or terminal illness across multiple counties. Acting as a leader within the team, the Senior Case Manager supports new hires, stabilizes cases, and advocates for individuals to achieve personal goals through community resources, in alignment with Accord's mission of helping people live their greatest lives.

Essential Responsibilities:
  • Listen-Learn-Lead: Model the mission and values of Accord, helping people to live their greatest lives.
  • Manage temporary cases, making corrections to case work, or completing work that was not finished. Provide coverage to cases as a result of turnover to ensure continuity of care.
  • Provide person-centered case management services to individuals of all ages with chronic illness, mental health issues, or life-altering conditions, helping them to gain access to rehabilitation programs, waiver and State plan services, and other community resources. Requires cross county training to work across multiple counties
  • Advocate for persons supported, working to achieve individual goals and quality outcomes through the use of community resources and empowerment.
  • Assist with new County Transfers by completing the initial visit, ensuring services are set up, creating a support plan and obtaining necessary signature pages.
  • Act as a leader on the team, offering shadow visits to support new employees and help them develop knowledge and skills
  • Prioritize and complete any missing or unfinished tasks related to a person supported during program transitions. Work to discharge or reassign the case once it has been stabilized and brought up to date. Handle complex cases requiring advanced case management expertise and focused attention.
  • Interact professionally with interdisciplinary team members including the person supported, family caregivers and guardians, employers, case managers, community providers, and other involved stakeholders.
  • Meet face-to-face at a minimum of 2 times per year and remotely with individuals and their support teams as needed to assess physical and mental wellness, needs, preferences, and abilities; apply information to develop tailored support plans.
  • Assist individuals to implement personal goals and support plans and access needed services to support these goals.
  • Provide ongoing review and evaluation of support plans and allocations, monitor the services identified in the plan, and adapt the plan as the needs, preferences, and waiver status change.
  • Monitor Medicaid eligibility status on a regular basis and provide reminders to the person supported or their team as needed to ensure benefits remain active to be eligible for the waiver.
  • Manage allocations and authorize services within the given budget, ensuring that duplication of services does not occur.
  • Manage a flexible caseload of approximately 50 people, across multiple counties. Case load size may be higher during seasons of increased turnover.
  • Attend all required annual and other meetings, demonstrating a high level of professionalism and genuine care for the person supported.
  • Practice customer service excellence and prompt responses within one business day for all communication, including written and verbal correspondence.
  • Demonstrate strong written communication skills; complete all required paperwork to comply with contractors and funding sources.
  • Complete documentation of all services provided in the selected electronic health record/database system within one business day of providing services.
  • Proven record of consistently exceeding minimum billing standards.
  • As a mandated reporter, respond and report all incidents that occur while providing services to protect the health and safety of and minimize risk of harm to the person(s) supported following Accord's policy and MN Statutes.
  • Represent the agency to the people we support, our community partners and external constituents. Serve as a positive role model for other employees.
  • Meet completion deadlines for all required and assigned training related to the position.
  • Comply with all Accord policies and procedures, rules and regulations of the State of Minnesota, and requirements of any contracted or funding parties.
  • Travel throughout the Twin Cities Metropolitan area to job sites and meeting locations; limited overnight travel possible.
  • Perform other duties as required and/or assigned.

Requirements
Education:
  • Bachelor's Degree in social work, nursing, psychology, sociology or a closely related field from an accredited four-year college or university; OR

A bachelor's degree from an accredited four-year college or university in a field other than social work, nursing, psychology, sociology or a closed related field AND one year of experience in the delivery of social services to the target population as a supervised social worker in a public or private social services agency.
  • If you hold a Social Work degree, we expect you to be licensed by the Minnesota Board of Social Work, as required.
