We are conducting a search for a growing private duty home care organization seeking a Home Care Liaison to drive referral growth throughout New York City and Nassau County.

Position Summary

This is a field-based business development role responsible for generating new private-pay referrals throughout New York City and Nassau County.

You'll spend the majority of your time in the field developing relationships with hospitals, assisted living communities, independent living communities, physicians, case managers, discharge planners, elder law attorneys, and other referral partners. The ideal candidate is a true hunter who enjoys opening new doors, building long-term partnerships, and consistently growing census.

Key Responsibilities

  • Develop and maintain referral relationships throughout New York City and Nassau County.

  • Generate new private-pay home care referrals through hospitals, assisted living communities, senior living providers, physicians, case managers, and community partners.

  • Execute consistent face-to-face sales calls and networking activities within the assigned territory.

  • Build long-term referral partnerships that produce recurring business.

  • Represent the organization at community events and professional networking opportunities.

  • Collaborate with clinical and operational teams to ensure an exceptional client experience from referral through admission.

  • Consistently achieve census and revenue growth goals.

Qualifications

  • Proven success in healthcare business development or outside sales.

  • Experience in private-duty home care, home health, hospice, senior living, assisted living, or another closely related post-acute setting.

  • Existing referral relationships within the New York City market are highly preferred.

  • Demonstrated hunter mentality with a track record of generating new business.

  • Strong communication, presentation, and relationship-building skills.

  • Ability to work independently while managing a large geographic territory.
