Job description

LHH seeks a bilingual ( Spanish ) Case Manager to support families experiencing homelessness in a family shelter setting. This is an opportunity to make a meaningful impact by helping families move toward permanent housing while providing advocacy, guidance, and connections to critical community resources.
What You'll Do
  • Conduct client intakes and comprehensive needs assessments
  • Develop and maintain Individualized Service Plans (ISPs)
  • Assist families with obtaining required documents and becoming housing-ready
  • Connect clients to housing, employment, financial, medical, mental health, and community resources
  • Complete program assessments and track client progress
  • Maintain timely case notes and accurate client records
  • Conduct follow-up reviews and ongoing service planning
  • Advocate for clients and help address barriers to housing stability
  • Maintain confidentiality and compliance with all applicable guidelines
  • Support families experiencing homelessness, including households with children
  • Work closely with a predominantly Spanish-speaking population
What We're Looking For
  • Fluent in both English and Spanish
  • A minimum of 1 year of previous case management experience
  • Experience working with homeless services, shelters, housing programs, family services, social services, or other vulnerable populations
  • Knowledgeable of harm reduction model
  • Knowledge of community resources and service navigation
  • Ability to build rapport and engage clients from diverse backgrounds
  • Compassionate, solutions-focused, and adaptable approach

Pay Details: $23.00 to $26.00 per hour
