The Care Liaison serves as a key support role within Solara Care Solutions, helping to build and maintain positive relationships with referral sources, clients, families, and community partners. Working under the direction of the Care Solutions Liaison, the Referral Coordinator assists with referral development activities, manages referral communications, coordinates community outreach efforts, and supports the growth of both Medicare-certified home health and private care services.

Qualifications:
Bachelor's degree in marketing, Business, Communications, Healthcare Administration, or related field (preferred).
Minimum of two (2) years of professional experience in healthcare marketing, business development, or related fieldโ€”home health experience strongly preferred.
Comprehensive understanding of Medicare home health regulations and reimbursement guidelines.
Proven success in relationship-building, referral management, and territory development.
Exceptional interpersonal, verbal, and written communication skills.
Proficiency in CRM platforms and Microsoft Office Suite.
Valid state driverโ€™s license and reliable transportation required.
