The Care Liaison serves as a key support role within Solara Care Solutions, helping to build and maintain positive relationships with referral sources, clients, families, and community partners. Working under the direction of the Care Solutions Liaison, the Referral Coordinator assists with referral development activities, manages referral communications, coordinates community outreach efforts, and supports the growth of both Medicare-certified home health and private care services.

Key Responsibilities:
  • Uphold and advance the mission, vision, and core values of Solara Care Solutions.
    · Protect and advocate for clients’ rights to dignity, independence, privacy, and fair treatment, ensuring prompt resolution of concerns.
    · Comply with all established safety standards and promptly report any unsafe conditions, incidents, or equipment concerns to management.
    · Develop, strengthen, and maintain strategic relationships with key referral sources and healthcare partners.
    · Identify and cultivate new referral opportunities to achieve growth and census objectives for both Medicare-certified home health and private duty services.
    · Provide up-to-date and accurate information to referral sources about Solara’s programs, services, and clinical advantages.
    · Achieve assigned sales, referral, and volume goals as defined by Solara leadership.
    · Referral Source Support.
    · Assist in developing and maintaining relationships with hospitals, physician offices, skilled nursing facilities, rehabilitation centers, senior living communities, and other referral partners.
    · Conduct regular outreach visits, phone calls, and follow-up communications with referral sources.
    · Serve as a point of contact for referral inquiries and assist in coordinating referral activities.
    · Deliver educational materials and service information to referral partners and community stakeholders.
    · Maintain referral contact databases and ensure accurate records of interactions.
    · Receive, track, and monitor incoming referrals to ensure timely follow-up and service initiation.
    · Coordinate with intake, clinical, and operational teams to facilitate smooth transitions of care.
    · Monitor referral status and communicate updates to referral sources as appropriate.
    · Assist with obtaining and organizing referral documentation when needed.
    · Ensure accurate tracking of referral outcomes and conversions.
    · Community Outreach and Marketing.
    · Assist with planning and coordinating community events, educational programs, health fairs, and networking activities.
    · Represent Solara Care Solutions at designated community functions and outreach events.
    · Support the distribution of marketing materials and promotional campaigns.
    · Maintain inventory of marketing and referral materials.
    · Assist in identifying potential referral opportunities within assigned territories.
    · Administrative and Reporting Responsibilities.
    · Maintain current knowledge of Solara services, programs, payer sources, and referral processes.
    · Participate in ongoing training related to healthcare marketing, customer service, and referral management.
    · Perform other duties as assigned.
Qualifications:
Bachelor's degree in marketing, Business, Communications, Healthcare Administration, or related field (preferred).
Minimum of two (2) years of professional experience in healthcare marketing, business development, or related field—home health experience strongly preferred.
Comprehensive understanding of Medicare home health regulations and reimbursement guidelines.
Proven success in relationship-building, referral management, and territory development.
Exceptional interpersonal, verbal, and written communication skills.
Proficiency in CRM platforms and Microsoft Office Suite.
Valid state driver’s license and reliable transportation required.
