• Conduct thorough patient assessments using standardized tools and personal interviews to develop and implement individualized treatment plans.
• Facilitate both individual and group therapy sessions, providing therapeutic support and guidance to patients and their families during emotional crises.
• Collaborate with multidisciplinary teams including doctors, nurses, and chaplains to create holistic care strategies that address physical, emotional, and spiritual needs.
• Regularly review and adjust treatment plans based on patient feedback and progress, maintaining clear documentation in our electronic health record (EHR) system.
• Educate patients and families about available resources, including grief counseling and support groups, and empower them with coping strategies.
• Stay updated on the latest evidence-based practices in social work and hospice care to ensure your services are grounded in the most current knowledge.

As a Licensed Clinical Social Worker at Reaching Beyond Limits, you will work primarily with patients in hospice care, providing emotional support to those facing terminal illnesses. This role requires a sensitive approach as you address complex emotional challenges, enhance communication between patients and their families, and assist them through the intricacies of end-of-life decisions. Your influence will extend to relatives, providing guidance and solace during their grieving process.

• A Master’s degree in Social Work (MSW) from an accredited institution.
• Current and valid LCSW license in the state of California.
• Demonstrated strong analytical skills and a compassionate demeanor when addressing sensitive issues.
• Excellent communication skills, both verbal and written, that facilitate effective interactions with patients and healthcare professionals alike.

• At least two years of experience in a clinical or therapeutic setting, preferably within hospice or palliative care environments.
• Proven track record in implementing successful treatment plans and providing psychotherapy.
• Experience with electronic health record (EHR) systems is preferred.
