Key Responsibilities

As a Licensed Clinical Social Worker at Reaching Beyond Limits, you will undertake a variety of critical tasks aimed at improving patient outcomes. Your responsibilities will include:

  • Conducting comprehensive patient assessments to understand their emotional, social, and financial needs, using evidence-based frameworks.
  • Designing and implementing individualized treatment plans that address complex psychosocial issues inherent in end-of-life care.
  • Providing therapeutic interventions through individual and family counseling, helping clients to cope with grief, loss, and other life transitions.
  • Facilitating support groups for families and caregivers of hospice patients, creating a space for sharing experiences and coping strategies.
  • Collaborating closely with interdisciplinary teams, including physicians, nurses, and spiritual care staff, to provide well-rounded care.
  • Documenting all interactions, treatment plans, and progress notes in compliance with organizational policies and state regulations.
Patient Population / Client Focus

Your role will focus primarily on patients receiving hospice and palliative care, many of whom face terminal illnesses and end-of-life challenges. You will engage with a diverse range of clients, each with unique family dynamics and emotional needs, requiring you to adapt your approach accordingly. Qualifications

To qualify for the Senior Licensed Clinical Social Worker position, candidates must possess:

  • A current and valid LCSW license in the state of California.
  • A Master’s Degree in Social Work from an accredited institution.
  • A minimum of two years of clinical experience working with patients in a healthcare or hospice setting.
Preferred Experience
