Key Responsibilities

As a Licensed Clinical Social Worker at Reaching Beyond Limits, you will undertake a variety of critical tasks aimed at improving patient outcomes. Your responsibilities will include:

• Conducting comprehensive patient assessments to understand their emotional, social, and financial needs, using evidence-based frameworks.
• Designing and implementing individualized treatment plans that address complex psychosocial issues inherent in end-of-life care.
• Providing therapeutic interventions through individual and family counseling, helping clients to cope with grief, loss, and other life transitions.
• Facilitating support groups for families and caregivers of hospice patients, creating a space for sharing experiences and coping strategies.
• Collaborating closely with interdisciplinary teams, including physicians, nurses, and spiritual care staff, to provide well-rounded care.
• Documenting all interactions, treatment plans, and progress notes in compliance with organizational policies and state regulations.

Patient Population / Client Focus

Your role will focus primarily on patients receiving hospice and palliative care, many of whom face terminal illnesses and end-of-life challenges. You will engage with a diverse range of clients, each with unique family dynamics and emotional needs, requiring you to adapt your approach accordingly. Your empathetic engagement will play a crucial role in helping families navigate one of life’s most difficult journeys.

Qualifications

To qualify for the Senior Licensed Clinical Social Worker position, candidates must possess:

• A current and valid LCSW license in the state of California.
• A Master’s Degree in Social Work from an accredited institution.
• A minimum of two years of clinical experience working with patients in a healthcare or hospice setting.

Preferred Experience

Ideal candidates will demonstrate strong problem-solving abilities and exceptional communication skills. Compensation and Benefits
