Social Worker Responsibilities & Duties

  • Conduct psychosocial assessments for new residents
  • Develop individualized care plans in collaboration with healthcare teams
  • Provide individual and group counseling to residents
  • Assist residents in adjusting to nursing home life
  • Advocate for residents' rights and needs
  • Facilitate communication between residents, families, and staff
  • Coordinate discharge planning and follow-up services
  • Assist families with understanding and navigating the healthcare system
  • Organize and implement recreational and therapeutic activities
  • Maintain accurate documentation and case records
  • Participate in interdisciplinary team meetings
  • Offer crisis intervention services as needed
  • Provide support and resources for end-of-life care
