Nassau County, NY

Pay: $50.00 per hour

Exciting New Opportunity with Affirmed Home Care!
Affirmed Home Care is seeking a compassionate and motivated per diem Medical Social Worker with NYS LMSW or LCSW licensure to join our team in Nassau County. This exciting new position provides rotating onsite social work coverage across three hotel-based client service locations( Syosset, Planview and Woodbury) . The Social Worker will play a key role in supporting clients through benefits navigation, housing coordination, care coordination, behavioral health support, discharge and hospital coordination, and community resource linkage, while also assisting clients in accessing food assistance programs and transportation services to help promote long-term stability and independence.
The Medical Social Worker will be responsible for:
  • Assisting clients with SSI/SSD applications
  • Supporting Medicaid enrollment and recertification
  • Assisting with SNAP applications and other public assistance programs
  • Connecting clients with food assistance programs and transportation resources
  • Providing guidance and support with housing applications
  • Assisting with transition planning to permanent housing
  • Collaborating closely with DSS case managers and other community partners
  • Coordinating discharge planning and hospital transitions
  • Providing mental health referrals
  • Coordinating substance abuse treatment referrals
  • Delivering crisis intervention and immediate support services
  • Linking clients to appropriate community-based resources
  • Advocating for client needs and ensuring access to appropriate support services

What We're Looking For
  • Active New York State LMSW or LCSW license
  • Master's Degree in Social Work (MSW)
  • 1+ year of experience in healthcare or social services (home care preferred)
  • Strong case management and care coordination skills
  • Excellent communication and interpersonal skills
  • Ability to manage caseloads in a fast-paced environment
  • Compassionate, patient-focused approach to care
