Work Environment

This position offers the flexibility of remote work, allowing our Licensed Clinical Social Worker the opportunity to provide vital support from home while still engaging with our clients and team through virtual consultations. Our clinic operates from Monday to Friday, with core working hours from 8:00 AM to 4:30 PM. We encourage flexible scheduling to accommodate individual client visits, and while remote, on-call availability may be required to address urgent client needs during off-hours.

Key Responsibilities

The role of the Licensed Clinical Social Worker involves numerous responsibilities designed to facilitate the overall well-being of our senior clients. Key tasks include:

  • Conducting comprehensive psychosocial assessments to evaluate the emotional, mental, and social needs of clients.
  • Collaborating with clients and their families to develop personalized care plans that focus on specific goals, preferences, and circumstances.
  • Providing individual and family counseling to help seniors and their loved ones adequately address and cope with life transitions.
  • Advocating for clients’ rights by facilitating access to community resources and necessary services to support their quality of life.
  • Engaging in teamwork with interdisciplinary professionals including nurses and healthcare providers to coordinate comprehensive care.
  • Regularly monitoring clients’ well-being through virtual visits, assessing living conditions, and addressing any safety or comfort concerns.
  • Facilitating communication among family members to resolve conflicts and enhance collaborative care solutions.
  • Educating clients and their families about available community resources, support groups, and programs to enhance their social engagement.
  • Maintaining meticulous documentation of client assessments, care plans, and progress notes in compliance with healthcare regulations.

Patient Population / Client Focus

This position primarily focuses on the senior population within our community clinic, addressing the unique challenges they face through physical, emotional, and situational support. Qualifications
  • A Master’s degree in Social Work from an accredited institution.
  • Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
