We are seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our Adult Day Care Center team. The ideal candidate will be dedicated to supporting older adults and individuals with disabilities by providing psychosocial assessments, care planning,  counseling, and family support services in a warm and collaborative environment.
Responsibilities:
  • Conduct psychosocial assessments and develop individualized care plans
  • Provide emotional support and counseling to participants and families
  • Coordinate with interdisciplinary team members regarding participant needs. 
  • Assist with community resources, referrals, and benefits coordination. 
  • Maintain accurate and timely documentation
  • Participate in care conferences and family meetings. 
Required Qualifications:
Licensure
:
The individual must be a Licensed Bachelor Social Worker (LBSW), Licensed Graduate Social Worker (LMSW), or Licensed Certified Social Worker (LCSW) in the State of Maryland.
Education: They must hold at least a Bachelor’s Degree in Social Work from an accredited college or university.
