Overview
Social Worker (MSW) - $4,000 Sign-On Bonus (to be paid throughout the first year of employment).
Full-Time, Daylight shift of 8:30 a.m. to 5:00 p.m.

Lutheran SeniorLife provides a comprehensive array of community-based health and social service programs throughout western Pennsylvania.
Our services include LIFE (Living Independence for the Elderly); skilled nursing, home health; personal care; adoption & foster care; assisted living; adult day services; memory care; hospice; palliative care; residential living; affordable housing; home safety monitoring; Meals on Wheels; rehabilitation services and private duty.
LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.
Social Worker (MSW) is within an Interdisciplinary Care Team setting, incumbent promotes and maintains the mental and social health of enrolled participants through assessment, treatment, teaching and counseling. Provides basic casework and consultation for LIFE Program participants. Facilitates communication between participants, their family and the Care Team. Responsible for the implementation of social work care plan and coordination of social work with other services.
Qualifications
Social Worker (MSW) requirements include:
  • Master's Degree (MSW) in Social Work from an accredited school of social work.
  • Minimum of one (1) years' experience working with a frail or elderly population.
  • Thorough working knowledge of current community health practice for the frail elderly from direct service experience.
  • Working knowledge of the interdisciplinary model of care management.
  • Experience working with cognitively impaired seniors.
  • Working knowledge of computers, Microsoft Word and Excel required.
