Responsibilities

Under the Direction of the Director of Social Services, the responsibilities of the Discharge Planner/Case Manager is to manage the coordination, planning, and placement resolution for all inpatient admissions in the assigned service area. Work directly with the hospital staff, other agencies, medical consenters, caregivers, and Department of Family and Protective Services (DFPS) to facilitate and coordinate access to behavioral health services and community resources and to ensure timely and clinically appropriate discharge plans are developed and implemented.

Qualifications
  • Education and Training:  Bachelor's degree in social work or related field. Licensed if applicable in the state in which the hospital resides. Current BLS certification required. Crisis management certification per hospital policy and standards required.
  • Experience:  Two or more years of related social work or behavioral health experience.
