About the Opportunity:

The Clinical Social Worker - Case Manager provides comprehensive clinical and case management services to support the social, emotional, and behavioral needs of services members and families. This role combines therapeutic intervention with coordination of care, ensuring clients receive appropriate services, treatment, and community resources to improve overall functioning and quality of life.

Key Responsibilities:

Clinical Services

  • Conduct comprehensive biopsychosocial assessments to identify client strengths, needs, and barriers.
  • Develop and implement individualized treatment plans using evidence-based therapeutic interventions.
  • Provide clinical counseling, including individual, family, and/or group therapy as appropriate to licensure and setting.
  • Monitor and evaluate client progress, adjusting treatment plans as needed.

Case Management

  • Coordinate and oversee client services, including medical, behavioral health, housing, financial, and social support resources.
  • Facilitate referrals to community programs, mental health services, substance use treatment, or other needed supports.
  • Advocate for clients to ensure access to appropriate services and benefits.
  • Collaborate with multidisciplinary teams-physicians, nurses, therapists, educators, probation officers, and community partners- to ensure continuity of care.

Crisis Intervention & Support

  • Respond to crisis situations by conducting risk assessments, de-escalation, and safety planning.
  • Provide immediate support and connect clients to urgent resources as needed.

Documentation & Compliance

  • Maintain accurate, timely, and confidential documentation, including assessments, progress notes, and treatment plans.
  • Ensure compliance with organizational policies, state and federal regulations, and professional ethical standards.
  • Participate in team meetings, case conferences, supervision, and ongoing professional development.

Qualifications:

  • Master's degree in Social Work (MSW) from an accredited program.
  • Active clinical licensure (e.g., LCSW, LMSW, LICSW, or equivalent, depending on state).
  • Experience in case management, clinical counseling, or social services preferred.
  • Knowledge of mental health, trauma-informed care, crisis intervention, and community resource systems.
  • Strong skills in communication, documentation, organization, problem-solving, and advocacy.
  • Ability to work with diverse populations and maintain professional boundaries.

Preferred

  • Two or more years of experience in social work or case management.
  • Experience working in healthcare, community mental health, hospital, or crisis settings.
  • Knowledge of local community resources and social service systems.
  • Bilingual proficiency.
