Job Title: Behavioral Health Case Manager II (BHCM II)
Location: New Castle, DE 19720
Hourly Pay Rate: $28–$32/hour

Position Overview
We are seeking an experienced Behavioral Health Case Manager II (BHCM II) to provide professional behavioral health case management services in a psychiatric outpatient setting. The BHCM II will work with individuals who have developmental disabilities, mental illness, or emotional disorders to help them achieve greater social, emotional, economic, and physical stability and personal independence.
The ideal candidate will have strong experience in behavioral health, social work, case management, assessment, treatment planning, counseling, and coordination of services.
Key Responsibilities
Provide comprehensive behavioral health case management services in a psychiatric outpatient setting.
Conduct client assessments and identify individual behavioral health and human service needs.
Develop, implement, monitor, and evaluate individualized treatment and service plans.
Provide person-centered services designed to promote independence and stability.
Coordinate services and resources for individuals with mental illness, developmental disabilities, or emotional disorders.
Provide counseling, interviewing, and supportive services to clients.
Monitor client progress and make appropriate adjustments to treatment or service plans.
Assess and manage client risk and safety concerns.
Coordinate referrals to appropriate social service agencies and community resources.
Collaborate with treatment teams, healthcare providers, state agencies, and private organizations.
Provide consultative services to other state and private agencies as appropriate.
Maintain accurate and timely case documentation and records.
Support comprehensive patient care while maintaining the integrity of Delaware's behavioral health services.
Perform other related duties as assigned by the administrative or site supervisor.
Minimum Qualifications
Required:
Master's degree in Behavioral Science, Social Science, Social Work, or a related field.
Three years of case management experience, including:
Assessing client needs
Treatment/service planning
Developing and implementing service plans
Monitoring and evaluating services
Using counseling or interviewing techniques
Connecting clients with appropriate resources
OR/Additional Qualifying Experience:
Three years of experience providing clinical treatment or counseling, or determining eligibility for services or benefits as part of a treatment team.
 
Preferred Qualifications
Experience working in a psychiatric, behavioral health, or outpatient mental health setting.
Experience working with individuals with mental illness, developmental disabilities, or emotional disorders.
Knowledge of behavioral health case management practices and community resources.
Strong assessment, documentation, communication, and interpersonal skills.
Ability to work effectively with multidisciplinary treatment teams.
Strong understanding of person-centered care and client safety/risk management.
