The UW Medical Center-Northwest Social Work Departmenthas an outstanding opportunity for aSenior Social Workerto join us in our newCenter for Behavioral Health and Learningbuilding.

WORK SCHEDULE

Full-Time / 40 hours per week

Day Shift

POSITION HIGHLIGHTS

Partner with patients, their families and the health care team to address medically-related social and emotional needs and provide support and advocacy as these impact the patient's medical condition, treatment, recovery and safe transition from one environment to another

Be responsible for providing a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention and resource referral(s)

Partner with the patient/family and the health care team to coordinate care across the continuum, improving patient care through effective utilization of resources by transitioning from the acute care episode to the post-acute setting

ABOUT THE CENTER FOR BEHAVIORAL HEALTH AND LEARNING

The newCenter for Behavioral Health and Learning, opening on the UW Medical Center - Northwest campus in 2024, offers an unparalleled opportunity to transform mental health care and to improve mental health care training in our state. Located in North Seattle, the new facility will feature 75 longer-term adult psychiatry beds, 25 geriatric psychiatry beds, 50 medical/surgical beds focused on patients with psychiatric comorbidities, and a state-of-the-art neuromodulation program. It will also house an interdisciplinary training and workforce development program focused on preparing and supporting the next generation of behavioral health providers for Washington State. The six-story building will be a beautiful place to work with lots of windows, natural lighting and open-concept spaces and will offer amenities including a new kitchen and dining area for patients, staff and visitors.

PRIMARY JOB RESPONSIBILITIES

Provide specialty consultation to other health care staff and health care teams regarding psychosocially focused aspects of client/patient care

Provide psychosocial support to patients and families

Complete clinical assessments as per department standards for area of service

In collaboration with the multidisciplinary team, develop and implement safe and timely discharge plans

Provide care coordination for patients with complicated social and medical histories

Provide case management and counseling services in specialized programs

Make timely referrals to hospital and/community agencies, including those required by mandatory reporting laws such as child and vulnerable adult abuse

REQUIRED POSITION QUALIFICATIONS

A Master of Social Work Degree from a program accredited by the Council on Social Work Education and one year of full-time social work experience in a health care setting or equivalent. Experience can include a practicum placement as part of a social work training program and/or employment experience.

Within 90 days of hire must hold one of the following credentials issued by the state of Washington:
