Job description

Job Opportunity: Social Work Assistant

Our organization is excited to announce a fantastic opening for a Social Work Assistant within the Social Work Department at our medical center.

Work Schedule
  • Per Diem
  • Variable Day Shifts, including weekends and holidays, with an 8-hour day shift.
  • All shifts will take place on-site.
Position Highlights

In this role, you will collaborate with Social Workers (SW), Nurse Care Coordinators (NCCs), patients, their families, and the healthcare team to enhance the care progression for patients. You will support the SW and NCC team in managing the discharge planning process effectively.

Primary Job Responsibilities
  • Assist in placing patients in long-term care facilities based on information gathered from clinical staff.
  • Help the SW/NCC team coordinate home care needs, including referrals to home health, hospice, in-home services, or obtaining home Wound VAC.
  • Facilitate safe transitions for patients returning home or moving to other facilities, including finding outpatient non-institutional settings such as respite care, shelters, or housing.
  • Support patients and families with urgent resource needs, including emergency funds, food vouchers, and transportation assistance.
  • Respond promptly to referrals from SW/NCCs, keeping them updated on case progress, documenting in the EMR according to departmental standards, and ensuring clear communication during handoffs.
  • Assist SW/NCCs in identifying, researching, and arranging resources for families, including transportation, housing, financial aid, and safety net programs.
  • Under the guidance of the SW/NCC, prepare letters and documents to help families secure benefits, and manage the copying, faxing, and mailing of documents as needed.
  • Provide notary public services to patients and staff.
Required Qualifications
  • Bachelor's degree in Social Work or a related field with two years of experience in social work.
  • OR an Associate of Arts degree in behavioral sciences with four years of experience in social work.
  • Equivalent education and/or experience may be considered to meet minimum qualifications, except where legal requirements apply, such as licenses or certifications.
