As a Medical Social Worker with Premier Home Health Providers, you will shape how we care for patients in their homes, mentor a team of social workers, and help set the standard for compassionate, high-quality service across our organization.

Please note that this role is per diem with a rate of $100 - $120 per visit.
Responsibilities
  • Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
  • Recruit, onboard, and mentor social workers-providing clinical supervision, observing practice, and supporting their professional growth
  • Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
  • Partner with the Executive Director on the annual department budget and resource planning
  • Build strong knowledge of community resources and connect patients and referral sources to the right support
  • Plan in-service training and continuing education to keep the team's skills sharp
  • Contribute to quality improvement initiatives and help identify new ways to better serve our patients
  • Perform other duties as required for patient services

Requirements
  • A Master of Social Work (MSW) from an accredited program
  • An active California license as an LCSW
  • At least 3 years of management experience, preferably in home care
  • Strong knowledge of healthcare regulations, ACHC standards, and business operations
  • Excellent communication skills and the ability to build trust with patients, families, staff, and the community
  • A self-directed, organized, and flexible approach-you take initiative and work well with others
