• Conduct comprehensive clinical evaluations to assess patient mental health needs and develop appropriate treatment plans.
• Lead psychoeducational and process-oriented therapy groups, utilizing evidence-based practices to facilitate patient learning and growth.
• Manage all aspects of case management, including initiating discharge planning on the day of admission, and ensuring follow-up care transitions are seamless and effective.
• Facilitate crisis intervention within therapy sessions and the broader community, demonstrating effective de-escalation techniques and sound clinical judgment.
• Engage with patients and their families to ensure they understand and can effectively implement coping strategies and transition plans upon discharge.
• Collaborate with referral sources and outpatient providers to guarantee continuity of care, enhancing the overall patient experience.
• Stay current with documentation requirements, including initial assessments, SMART treatment plans, and clinical case notes, to ensure compliance with regulatory standards.
• Provide additional clinical support to team members as necessary, fostering a collaborative environment that empowers all clinical staff.
• Actively participate in clinical outreach and promotional programs to increase community awareness of mental health services.
• Assist milieu staff in their interactions with patients, providing guidance that exemplifies best practices in social work.

This role focuses on serving a diverse array of clients within the community health landscape, including individuals from varied cultural backgrounds, socioeconomic statuses, and age groups. As an LCSW, you will address mental health challenges ranging from anxiety and depression to complex trauma and behavioral issues. Your interventions will be critical in promoting overall wellness and enhancing the quality of life for our clients.

• Master’s degree in Social Work, Counseling, Psychology, or a related mental health discipline.
• Must hold an active, independent license as a Licensed Clinical Social Worker (LCSW) in the state of California.
• Demonstrated proficiency in written and verbal communication, with an emphasis on patient documentation and interactions.
• Strong ability to establish and maintain appropriate boundaries with clients, providing a safe and supportive therapeutic environment.
• Experience working with a diverse client base and demonstrating cultural competency.
• Proficient in utilizing Office 365 and Electronic Health Record systems to manage clinical documentation efficiently.

• Prior experience in a community health setting is highly desirable.
• Experience in crisis intervention and management, particularly in high-stress clinical situations.
• Familiarity with treatment modalities such as Dialectical Behavior Therapy (DBT) or Cognitive Behavioral Therapy (CBT) is a plus.

• Competitive annual salary ranging from $120,000 to $130,000 for full-time positions.
