Job description

Pay scale- $19.10-$21.00 Per Hour Based on Experience 
Qualifications:
  1. AA degree or 2 years working in psych/social environment-preferably with seniors
  2. Intermediate knowledge of Social Services functions
  3. Good verbal and written communication skills
  4. Goal and task oriented
  5. Ability to be self-directed, as well as, have good inter-personal skills
  6. Good problem solving and time management skills
  7. Ability to handle confidential data discreetly
Position Responsibilities and Duties:
  1. Regular and punctual attendance
  2. Report any, and all, Resident psychosocial changes or concerns immediately to Director of Social Services
  3. Perform timely documentation of Social Service interventions and assessments in PCC system
  4. Assist with coordinating smooth and safe Resident discharges into the community, Assisted Living, or the Retirement Center
  5. Assist with occurrences of theft/loss reports and provide any follow-up that may occur
  6. Assist with coordinating Resident room changes
  7. Participate in regularly scheduled Resident Care Plan Conferences, PPS meetings and other meetings as designated by Director of Social Services
  8. Notify new admissions of the time and date of care plan meetings by giving them written notice of the meeting at least 48 hours in advance
  9. At beginning of each month check eligibility and print report for those Resident's who are on Medi-cal; IEHP or Molina
  10. Assist with helping to cover Admissions department when the Admissions Director is out of the building (this would include but is not limited to: tours, inquiries, admission packets)
