Job description

JOB PURPOSE:   
Assists the Social Services Director with the day-to-day activities of social services within the building assigned. In accordance with the company policy and all applicable State and Federal laws, regulations and professional guidelines.
 
KEY RESPONSIBILITIES:
  1. Possesses and demonstrates working knowledge and ability to apply standards of professional social work practice in long term care setting.
  2. Develops and maintains social work policies and procedures reflecting current professional standards and philosophy of care consistent with State and Federal regulations.
  3. Communicates directly with residents, family, staff, interdisciplinary team members and Department Heads to coordinate care and services, improve organization and implementation of plans of care to maintain a high quality of care.
  4. Demonstrates knowledge and respect for the rights, dignity and individuality of each resident in all interactions.
  5. Demonstrates competency in the protection and promotion of and advocacy for residents’ rights.
  6. Possesses thorough knowledge and understanding of quality improvement processes as well as emergency and safety requirements.
  7. Responds to resident behavioral and psychiatric issues by completing behavioral and psychosocial assessments, providing treatment recommendations and making referrals to appropriate mental and behavioral health providers.
  8. When necessary or assigned, maintains a social services caseload; providing social services to residents as required.
  9. Consult on a regular basis with the facility’s Behavior Management Program to ensure that the program is properly implemented and functioning to meet the psychosocial needs of the resident.
  10. Able to understand and follow written and verbal directions.
  11. Demonstrates honesty and integrity at all times.
  12. Demonstrates knowledge of emergency and disaster protocols.
  13. Demonstrates ability to prioritize responsibilities and complete projects within the allotted time and within the allotted budget.
  14. Able to respond to change productively and to handle additional projects as assigned.
  15. Maintain skills and knowledge through continuing education Applies information to job responsibilities.
MINIMUM EDUCATION REQUIRED:
High School diploma or equivalent.
 
MINIMUM EXPERIENCE REQUIRED:
One (1) year experience in social work.
 
ADDITIONAL QUALIFICATIONS: (Preferred qualifications)
A degree in Social Work or related field is preferred.
