Symphony Applewood is seeking a compassionate and dedicated Social Service Director to support our short-term rehab and long-term care guests. This role is responsible for providing psychosocial support, discharge planning, advocacy, and care coordination while working closely with residents, families, and the interdisciplinary team.

Responsibilities
  • Conduct psychosocial and cognitive assessments
  • Develop and maintain resident care plans
  • Assist with discharge planning and transitions of care
  • Provide emotional support and advocacy for residents and families
  • Collaborate with nursing, physicians, and interdisciplinary team members
  • Maintain accurate documentation and participate in care conferences
  • Support end-of-life, hospice, and palliative care discussions

Requirements

Qualifications & Experience
  • Bachelor's Degree required
  • Minimum 2-3 years of experience in long-term care and short-term rehabilitation preferred
  • Strong conflict resolution and problem-solving skills
  • Excellent communication and interpersonal abilities
  • Knowledge of federal and state regulations related to long-term care
  • Strong organizational and time management skills
  • Compassionate, professional, and empathetic demeanor
  • Knowledge of PointClickCare preferred
