Key Responsibilities

  • Develop and implement individualized service plans that clearly outline specific, measurable goals and interventions tailored to each clientโ€™s unique needs.
  • Provide therapeutic counseling to individuals and families dealing with grief, loss, and adjustment to life-limiting illness, using evidence-based therapeutic techniques.
  • Engage clients in the development of personalized care plans, ensuring they feel a sense of ownership in their path to healing and empowerment.
  • Offer immediate crisis intervention, risk assessments, and safety planning to manage acute mental health concerns as they arise.
  • Collaborate with a network of community resources and services, advocating for clients as they navigate challenges and secure the support they require.
  • Participate in community outreach efforts aimed at increasing awareness of hospice resources and enhancing access for under-served populations.
  • Contribute to case conferences, attend team meetings, and engage in ongoing staff training to ensure alignment with best practices and agency mission.
Patient Population / Client Focus

The ideal candidate will work primarily with individuals and families facing end-of-life issues at our hospice center, often addressing topics such as terminal illnesses, bereavement, and existential challenges. Our multidisciplinary approach involves working alongside healthcare professionals to ensure holistic care is provided.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited university is required.
  • Current, active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Colorado.
  • A strong understanding of ethical practices in social work, particularly in palliative and hospice care situations.
Preferred Experience
  • At least one year of experience working within a social service agency or hospice setting is preferred.
