Position Summary

We are seeking a compassionate and motivated Social Services Assistant to support our Social Services Department. This role plays a key part in assisting residents and families with psychosocial needs, discharge planning, and care coordination in a long-term care / nursing home setting.


Key Responsibilities
  • Assist the Social Services Director in providing social services support to residents and families
  • Help coordinate discharge planning and transitions of care
  • Support residents with psychosocial needs, emotional support, and adjustment to facility life
  • Maintain accurate documentation in accordance with CMS and Florida state regulations
  • Participate in care plan meetings and interdisciplinary team (IDT) discussions
  • Assist with resident advocacy, grievance processes, and family communication
  • Coordinate community resources and referrals as needed
  • Ensure compliance with HIPAA and resident rights regulations

Qualifications
  • Experience in social services, healthcare, or long-term care preferred
  • Bachelor’s degree in Social Work, Human Services, Psychology, or related field preferred
  • Knowledge of skilled nursing facility (SNF) operations is a plus
  • Strong communication, organizational, and interpersonal skills
  • Ability to work effectively with residents, families, and interdisciplinary teams
