• Act as a crucial liaison between families and outpatient providers, promoting cohesive and coordinated care.
• Engage with insurance companies to facilitate pre-certification processes and manage utilization effectively, ensuring that clients have access to the necessary services.
• Participate in planning client discharges and aftercare, delivering a seamless transition back into the community with appropriate support.
• Provide therapeutic meal support, modeling healthy relationships with food and addressing emotional well-being related to nutrition.
• Deliver safe, empathetic, and affirming care tailored to each client's unique circumstances, taking into account their intersecting identities for a holistic treatment approach.
