As a member of our Hospice Center, you will work closely with a multidisciplinary team to create therapeutic environments for recovery. As a Licensed Clinical Social Worker, your contributions will include:

• Conducting thorough assessments to determine the counseling needs of clients, taking into account their unique circumstances and backgrounds.
• Utilizing DSM (Diagnostic and Statistical Manual) criteria to assess clients for substance abuse diagnoses and determining appropriate levels of care based on ASAM (American Society of Addiction Medicine) guidelines.
• Developing comprehensive treatment plans that address the substance abuse and mental health needs of clients, integrating their goals into actionable strategies.
• Crafting discharge and continuum of care plans to ensure smooth transitions to the next appropriate level of service.
• Maintaining meticulous documentation of all counseling services, treatment plans, assessments, and follow-ups within the clients' clinical records to comply with regulatory standards.
• Managing a caseload of clients, including case management and utilization review updates, ensuring adherence to treatment protocols.
• Coordinating support services for clients and their families, including documentation of provided services to aid in comprehensive care.
• Actively communicating with referral sources (e.g., legal representatives, schools, employers) to create a network of support for clients.
• Employing crisis intervention techniques effectively, using a multidisciplinary team approach to address urgent client needs.

Your work will predominantly involve supporting individuals facing co-occurring mental health and substance use disorders. Your interventions will play a vital role in promoting long-term recovery, providing clients with the tools and skills necessary for sustainable change. To be successful in this role, you must possess:
