Job description

Job Summary

We are seeking a dedicated and compassionate Registered Nurse (RN) to provide skilled home health visits to patients in their homes. This position begins as part-time and is ideal for an RN who enjoys independent work, flexible scheduling, and one-on-one patient care. Driving to patient homes is required.

Responsibilities

  • Provide skilled nursing care in patients’ homes per physician orders and care plans

  • Perform comprehensive assessments, vital signs, and skilled nursing interventions

  • Administer medications, wound care, injections, and treatments

  • Educate patients and caregivers on medications, disease processes, and safety

  • Monitor patient progress and report changes in condition

  • Complete accurate, timely documentation for each visit

  • Communicate with physicians, case managers, and interdisciplinary team members

  • Follow all home health regulations and agency policies

Qualifications

  • Active Massachusetts Registered Nurse (RN) license in good standing

  • Minimum 1 year of RN experience (home health experience preferred, not required)
