Admissions RN

Connected HH is seeking a full-time, hourly Home Health Admissions RN to support our Portland Metropolitan service area. This role is ideal for nurses who excel in first-visit assessments, care-plan initiation, and establishing a strong foundation for each patient's home-health episode.

This position follows standard business hours, Monday through Friday, providing predictable scheduling, strong interdisciplinary support, and a balanced workflow.

Key Responsibilities
  • Complete comprehensive Start-of-Care (SOC) nursing assessments in patient homes.

  • Initiate individualized care plans and establish expectations for the home-health episode.

  • Conduct medication reconciliation, safety assessments, and disease-specific education.

  • Coordinate care with interdisciplinary team members to ensure timely service initiation.

  • Provide skilled nursing interventions as needed during SOC visits.

  • Maintain accurate, timely documentation in accordance with regulatory and organizational standards.

  • Communicate effectively with physicians, caregivers, and internal team members.

  • Support safe transitions from hospitals, SNFs, and outpatient settings.

Schedule
  • Mondayโ€“Friday

  • Normal business hours (daytime schedule)

  • No evenings, nights, or rotating weekends

  • Predictable workflow with full team availability during working hours

Qualifications
  • Current Oregon RN license (or ability to obtain).

  • One year of nursing experience preferred; home-health or admissions experience a plus.

  • Strong assessment, communication, and critical-thinking skills.

  • Ability to work independently in a field-based role.

  • Reliable transportation and valid driver's license.
